IgM anti-HBc positivity identifies acute hepatitis B rather than chronic infection.

Side by side

Hepatitis B acuteHepatitis B chronic
Key clue

Recent hepatitis syndrome with positive HBsAg and IgM anti-HBc indicating newly acquired infection.

Persistent HBsAg for more than 6 months with IgG anti-HBc, with or without elevated HBV DNA and ALT.

Mechanism

Acute immune response to newly infected hepatocytes produces transient hepatocellular injury and serologic IgM response.

Failure to clear HBV leads to persistent viral replication or immune-active inflammation with long-term fibrosis and cancer risk.

Labs and tests

HBsAg positive with IgM anti-HBc positive, ALT and AST often in the hundreds to thousands U/L, and INR should be followed to detect evolving acute liver failure.

HBsAg remains positive beyond 6 months with IgG anti-HBc, HBV DNA level quantifies replication, and treatment is considered when HBV DNA is elevated with persistent ALT elevation or cirrhosis.

The trap

Clinicians sometimes mislabel chronic HBV flare as acute infection based only on high transaminases. The resolving clue is core antibody class and time course: IgM anti-HBc supports acute infection, while persistent HBsAg beyond 6 months with IgG anti-HBc supports chronic disease that needs fibrosis and antiviral treatment assessment.

The repair move

Next best step: provide supportive care and check INR, bilirubin, and mental status to screen for acute liver failure. Avoid unnecessary antivirals in uncomplicated acute HBV, but if there is severe disease or liver failure concern, start nucleos(t)ide therapy such as tenofovir while arranging urgent hepatology involvement. For chronic HBV, stage fibrosis and start long-term antiviral therapy when criteria are met.

Core

A 28-year-old man presents with jaundice and fatigue for 10 days. BP 120/78 mm Hg, HR 88/min, temp 37.4 C. Labs show ALT 1100 U/L, HBsAg positive, and IgM anti-HBc positive. What is the next best step in management?

Show answer and explanation

Answer: B. Supportive care with close monitoring for acute liver failure signs. Most acute HBV infections are managed supportively, with close monitoring for decompensation rather than routine immediate long-term antiviral therapy.

Why the other choices are tempting

  • A. Wrong. Universal immediate indefinite antiviral therapy is not required in uncomplicated acute HBV.
  • C. Wrong. IgM anti-HBc indicates acute, not chronic, infection.
  • D. Wrong. Steroids are not standard treatment for uncomplicated acute viral hepatitis B.
  • E. Wrong. Follow-up is essential to confirm recovery and serologic clearance.
