Schizophrenia and schizophreniform disorder vs Brief psychotic disorder and schizoaffective disorder

How to tell Schizophrenia and schizophreniform disorder and Brief psychotic disorder and schizoaffective disorder apart when a board question makes them look alike.

Fastest separator

Schizophrenia is 6 months or more; schizophreniform is 1-6 months; brief psychotic is under 1 month; schizoaffective requires mood episodes plus 2 weeks psychosis alone.

Side by side

Schizophrenia and schizophreniform disorderBrief psychotic disorder and schizoaffective disorder
Key clue

Schizophrenia requires at least 6 months of illness with active psychotic symptoms and functional decline; schizophreniform has the same symptom pattern but lasts 1-6 months.

Brief psychotic disorder lasts 1 day to 1 month with return to baseline; schizoaffective disorder has psychosis plus mood episodes with at least 2 weeks of psychosis without mood symptoms.

Mechanism

Primary psychotic disorder is classified mainly by duration and functional impact after excluding substances, medical causes, and mood-driven psychosis.

Brief psychosis is time-limited; schizoaffective disorder combines a schizophrenia-spectrum psychosis with substantial mood episodes.

Labs and tests

Schizophrenia (DURATION 6 months or more): at least 2 of 5 symptom categories for at least 1 month (delusions, hallucinations, disorganized speech, grossly disorganized or catatonic behavior, negative symptoms), with one being delusions, hallucinations, or disorganized speech; psychosis plus social/occupational dysfunction over 6 months. Schizophreniform disorder: identical to schizophrenia but DURATION 1 to 6 months; about a third progress to schizophrenia, a third resolve completely, a third evolve into a mood disorder.

Brief psychotic disorder: psychotic symptoms lasting LESS than 1 month with full return to baseline, often triggered by major stressor (postpartum, bereavement). Schizoaffective disorder: meets criteria for schizophrenia plus a major mood episode (manic or depressive), but with at least 2 weeks of PSYCHOSIS WITHOUT prominent mood symptoms during the lifetime of the illness; if mood symptoms are always present during psychosis, it is bipolar with psychotic features or major depression with psychotic features, not schizoaffective.

The trap

Psychotic symptoms overlap, so duration and relationship to mood episodes decide the diagnosis.

The repair move

For psychotic disorders, first time the episode, then ask whether mood symptoms fully explain psychosis.
