Paget disease of bone vs Osteoporosis and osteomalacia

How to tell Paget disease of bone and Osteoporosis and osteomalacia apart when a board question makes them look alike.

Fastest separator

Paget is focal high-turnover disorganized bone with high ALP; osteoporosis is low mass with normal mineral labs; osteomalacia is poor mineralization with vitamin D/phosphate clues.

Side by side

Paget disease of boneOsteoporosis and osteomalacia
Key clue

Focal disordered bone remodeling with high alkaline phosphatase, bone pain, skull enlargement, hearing loss, deformity, and mosaic lamellar bone.

Osteoporosis causes low bone mass with normal mineralization and fragility fractures; osteomalacia causes defective mineralization with bone pain, weakness, and vitamin D/phosphate problems.

Mechanism

Excess osteoclast resorption followed by disorganized osteoblast repair creates enlarged but weak woven/mosaic bone.

Osteoporosis loses bone quantity with normal mineral; osteomalacia fails to mineralize osteoid because vitamin D or phosphate availability is inadequate.

Labs and tests

Isolated high alkaline phosphatase with normal calcium/phosphate and focal bone changes supports Paget disease.

Normal calcium/phosphate/ALP can occur in osteoporosis; low vitamin D, low phosphate or calcium, and high ALP support osteomalacia.

The trap

All three cause fractures or bone pain, but the lab panel and mechanism differ. PAGET disease is focal high-turnover disorganized bone (overactive osteoclasts followed by reactive osteoblastic mosaic pattern) in older adults; calcium NORMAL, phosphate NORMAL, PTH NORMAL, but alkaline phosphatase MARKEDLY ELEVATED (often above 2 to 3 times upper limit); presents with bone pain, increased hat size, hearing loss from cranial nerve compression, high-output cardiac failure from increased bone vascularity, osteosarcoma risk (low absolute risk but high relative); treat with bisphosphonate. OSTEOPOROSIS is decreased bone mass with normal mineralization in postmenopausal women or elderly men; ALL labs NORMAL (calcium, phosphate, ALP, PTH); diagnose by DEXA T-score under -2.5. OSTEOMALACIA / RICKETS is defective mineralization from vitamin D or phosphate deficiency; LOW calcium and phosphate (or normal/low), HIGH PTH, HIGH alkaline phosphatase, LOW 25-hydroxyvitamin D; bone pain, proximal myopathy, fractures, rickets in growing children (bowed legs, rachitic rosary, frontal bossing).

The repair move

For bone disease, decide focal remodeling, low bone mass, or defective mineralization.

Core

An older adult has skull enlargement, hearing loss, bone pain, normal calcium and phosphate, and isolated elevated alkaline phosphatase. Biopsy shows a mosaic pattern of lamellar bone. Which diagnosis is most likely?

Show answer and explanation

Answer: A. Paget disease of bone. Focal bone pain with skull enlargement, hearing loss, isolated ALP elevation, and mosaic bone points to Paget disease.

Why the other choices are tempting

  • B. Osteoporosis causes low bone mass with normal mineralization and often normal labs.
  • C. Osteomalacia causes defective mineralization with vitamin D/phosphate abnormalities.
  • D. Multiple myeloma causes lytic lesions, anemia, renal dysfunction, and monoclonal protein clues.
  • E. Osteopetrosis causes dense brittle bone from osteoclast dysfunction.
